Introduction

 

As an independently owned Physical Therapy Private Practice, the Owners are directly involved to ensure that every aspect of a patient’s experience, while attending Fowlerville Physical Therapy Specialists is excellent, including the clinical care that they receive.

 

We specialize in the rehabilitation & prevention of orthopedic & overuse / sports injuries & conditions for ALL ages, while we are best recognized for our expertise & success treating spinal related problems. In addition, our Therapists have received significant advanced training & certifications over the years which enables us to provide a no. of specialty services.

To schedule a patient for a Physical Therapy is easy & convenient.

 

1. Simply call our Fowlerville clinic at 517-223-8308 or our Howell clinic at 517-545-3200 and one of our front desk staff will be available to assist you

 

OR

 

2. If you prefer, you can fax their referral to our Fowlerville clinic at 517-223-8344 or our Howell clinic at 517-545-3236 and include their phone no. & we will contact them to schedule their initial appointment.

 

If you have their insurance information, this is helpful. Also if, their condition or injury is auto or work related, it is helpful if you can provide us with any authorization information that you may have.  However, we can also obtain this information ourselves, when we contact the patient.
  
Insurances

 

We are a Physical Therapy Provider for all principal insurance plans that are offered in Livingston County & surrounding areas.  Also our clinic is part of the Physical Therapy Provider Network (PTPN) which enables us to be a provider for several National & lesser known insurance plans.

 

In addition, we are also pleased to provide therapy for patients that are involved in an auto or work related injury, after we receive authorization from their Adjuster and/or Case Manager.
